Registration Form 報名表格

To Register 登记说明 :

  1. This program is run on Zoom so your child must have access to a computer or phone with a working microphone and webcam to participate. 此程序喺Zoom上運行,因此您的孩子必須有權訪問電腦或手機與工作咪同網絡攝像頭參與。

  2. Fill out the registration form. | 填寫登記表。

  3. Send forms and payment. | 發送表格和付款。

  4. Please send a digital copy of the form to alpha4youth@gmail.com and if form is handwritten please include the original when you drop off your payment. 請將表格的PDF副本alpha4youth@gmail.com 如果表格是手寫的,請在付款時包括原件。

  5. Please make cheques payable to “Living Stone Assembly,” and include student names & number of total sessions. 支票抬頭: Living Stone Assembly , 在支票上請注明學生名字及會話總數。
